Job Description

This position works closely with pharmacy practitioners as well as other health care professionals to ensure patient prescribed medications contribute to healthy outcomes. The incumbent provides direct patient care services that assess medication related needs, communicates care plans with patients and health care providers, and instructs pharmacy students on practical experiences. This position reports to the Associate Dean for Clinical Affairs.

Job Responsibilities

Provides direct patient care services. (i.e. patient assessment, care plan development, medication therapy, follow-up evaluation).

Delivers services in person or through telehealth technology.

Prepares reports documenting health care services provided.

Builds and maintains relationships with patients and health care providers.

Assesses patient’s health care problems and determines which prescribed medications optimally meet the intended goals of care.

Evaluates effectiveness of patient’s medications.

Reviews records of patients to determine appropriateness of medication therapy.

Develops care plans to educate patients on improving and maintaining health.

Communicates care plans with patients and health care providers.

Provides follow-up assessment to determine effects of medications on patient’s health.

Creates and maintains clinical protocols related to project work.

Instructs pharmacy students through clinical and practical experiences.

Performs related or similar duties as required or assigned.

Essential Functions

1. Provides direct patient care services that assess medication related needs.

2. Communicates care plans with patients and health care providers.

Minimum Qualifications

Education:
Doctor of Pharmacy from an accredited college or university.

AND

License:
Current license from the Mississippi State Board of Pharmacy.
